This notice explains the contact form and scheduling tools on the More Life Assurance website. For questions about your information, contact Shaun Adams at info@morelifeassurance.com.
Information you share
The contact form asks for your name, service interests, contact preference, and permission to contact you. Provide an email address if you prefer email, or a phone number if you prefer text. The other contact method, coverage details, meeting preference, and additional questions are optional. We do not ask for your address or date of birth on this initial form. If further details are needed, we’ll discuss how to provide them during follow-up. Any information you type into the additional questions box is included with your submission.
Why we ask
Your submission provides the information needed to respond to your inquiry, understand the topics you want to discuss, and coordinate an appointment. Permission to contact is required to submit the form. Submitting does not enroll you in insurance or obligate you to purchase a policy.
How the form and booking work
The form sends your information to FormSubmit, which processes it and emails it to Shaun Adams. A spam-prevention check may appear before you continue. After submission, you can open Calendly and enter the details needed to schedule an appointment. The website does not automatically pass your form answers to Calendly.
Cloudflare hosts and delivers this website and may process technical information associated with visits, such as IP addresses and request information. FormSubmit, its verification services, Calendly, and email providers process information as part of providing their services. Their own privacy notices explain their practices:
Permission and records
The submission includes the permission wording you accepted, your selected topics and contact method, and the version of the contact notice. Your permission is for Shaun Adams to respond about those topics. It is not permission for other agents or marketing organizations to contact you, and it is not a Medicare Scope of Appointment or enrollment application.
Your contact choices
Choose email or text for a response to your inquiry. Message and data rates may apply if you choose text. To change your preference or ask us to stop contacting you about your inquiry, email info@morelifeassurance.com.
Access, corrections, and removal requests
You can email the same address to ask about information you submitted, request a correction, or request removal. Include enough information to identify your inquiry, but do not email identity documents or sensitive account details. We may need to confirm your identity before acting on a request. Some records may need to be retained for legal or business recordkeeping purposes; a removal request does not automatically erase records held independently by other service providers.
Information to leave out
Please do not submit Social Security numbers, medical details, financial account numbers, or payment information through this form. This is an initial inquiry form, not an insurance application. No website or email service can guarantee absolute security.

Third-party pages used for form verification and scheduling may use cookies or similar technologies under their own policies. You can manage cookies through your browser and through the choices offered by those services.
